Job Description
**Responsibilities**
The Community Health Workers (CHWs) role will to be to conduct outreach to recruit faith-based community churches to act as community hubs for chronic disease prevention information dissemination.
Specific goals include:
+ Outreach and building relationships with faith-based community churches
+ Identifying a champion at each site and training each champion in the Diabetes Empowerment Education Program (DEEP) curriculum
+ Empower identified champions to lead DEEP workshops in their communities
+ Program goal: increase awareness, prevention, and education to African American, Hispanic, Native American, Asian, and Pacific Islander populations who are at risk of life-threatening kidney disease
****This position pays $23.00****

**Qualifications**
**Minimum:**
+ High School Diploma
+ Experience working in the community.
+ Experience with Community Organizations and Home Health follow-up
