Job Title: Health-Related Social Needs (HRSN) Screener/Navigator

Position Summary:

We are seeking a dedicated and detail-oriented Health-Related Social Needs (HRSN) Screener/Navigator to join our team. The candidate will be responsible for screening and navigating members for Health-Related Social Needs, ensuring compliance with all relevant processes and standards. This role involves direct interaction with community members, obtaining necessary consents, maintaining accurate records, and utilizing a specialized IT platform for data entry and reporting. The Screener/Navigator will contribute to efforts to advance health outcomes and reduce health disparities by providing case management and referrals for eligible Medicaid patients.

Essential Functions:

Screening and Navigation:

  • Provide empathetic and culturally competent screening for social determinants of health to community members telephonically, virtually, and/or in-person.
  • Process screens and referral intake daily including calls with potential program participants, documentation, and insurance/eligibility verification.
  • Receive, prioritize, monitor, and triage screenings and referrals that require service connections across community-based organizations.
  • Manage incoming referrals to ensure successful and timely connections are made between clients (community members) and the appropriate community partner that best meets community members’ needs.
  • Document progress and action taken with each referral and every interaction in the WNY Integrated Care IT platform (FindHelp), as required.
  • Maintain effective communication with community members and/or referring organizations to ensure acceptance and resolution of referrals by the receiving organization or to redirect the referral to another provider, if necessary.
  • Handle customer and other community organization interactions with excellent customer service, empathy, and patience.

Data Management & Reporting:

  • Enter data and interactions accurately, ensuring compliance with reporting standards.
  • Collaborate with the WNY Integrated Care Collaborative on performance reporting and data collection for regulatory compliance and performance evaluations.
  • Ensure timely submission of data and interactions.

Training & Compliance:

  • Abide by policies and procedures that support federal, state, and local HIPAA and Fraud, Waste, and Abuse regulations, as well as any other relevant regulations.
  • Maintain cultural and linguistic competency in screenings, following Culturally and Linguistically Appropriate Services (CLAS) Standards and completing relevant training.
  • Complete mandatory training on child abuse and neglect reporting (as per New York State requirements).
  • Attend WNY Integrated Care Screening/Navigation Workgroup.
  • Participate in other professional development and maintain certifications required by WSCS, the Western New York Integrated Care Collaborative, and any other relevant entities.

Technological Proficiency:

  • Utilize the WNY Integrated Care social care technology and transportation platforms.
  • Adapt to the use of new screening platforms and data reporting tools as necessary.
  • Work with live language interpreters using tools such as Boostlingo.

Languages & Accessibility:

  • Conduct screenings, assessments, and customer interactions via multiple formats including in-person, telephone, virtual, and/or text messaging.
  • Conduct screenings, assessments, and customer interactions in multiple languages with the assistance of live interpreters via phone call, app, and video tools.

Outreach

  • Establish additional locations for conducting screening by partnering with healthcare providers, schools, and other organizations.

Other Duties as Assigned

Qualifications:

  • Minimum bachelor’s degree in social work, public health, or a related field or equivalent job experience.
  • Experience providing care coordination and/or care management.
  • In compliance with federal law, all persons hired will be required to verify identity and eligibility to work in the United States.
  • Candidate must have a valid driver’s license, a clean driving record, and consistent daily access to a vehicle, as position requires outreach to sites around Western New York (mileage reimbursement provided for local travel).
  • Strong attention to detail and organizational skills.
  • Ability to maintain confidentiality and adhere to data privacy laws.
  • Understanding and commitment to cultural competency and providing services to diverse populations.
  • Completion of required training, including child abuse and neglect reporting certification.
  • Experience with HIPAA regulations is preferred.
  • Experience working with refugee and immigrant populations, as well as non-English speakers is preferred.
  • Multilingual candidates are strongly encouraged to apply. Priority languages in addition to English are Spanish, Arabic, and Burmese, but other languages are helpful.
  • Community health workers are strongly encouraged to apply.
  • Candidate must be eligible to work within Medicaid and Medicare programs (we will conduct exclusion checks monthly).
