Responsible for social work evaluation of patient/family psych/social and environmental needs, as well as implementation of the social work treatment plan and documentation consistent with insurance guidelines. Work is evaluated by Home Health Manager through patient record documentation, verbal communication and direct observation. Work is normally performed in a home setting. Requires considerable travel. Occasional work is performed in an office setting.

Required Qualifications:
Master's Degree - MSW from a school of Social Work.
Washington Advanced Social Worker License upon hire. Or
Washington Advanced Social Worker Associate License upon hire. Or
Washington Clinical Independent Social Worker License upon hire. Or
Washington Clinical Independent Social Worker Associate License upon hire.
Within 30 days of hire: National Provider Identifier (NPI) number
1 year of experience in a health care setting.
3 years of Post Master's work experience, or prior experience in hospice care setting.
Preferred Qualifications:
2 years of experience in a health care setting.
Prior experience in home health/hospice care setting.
